Certified copies are used for legal purposes and can be obtained from the town where the ceremony was performed.
Whatever your legal name is at the time of application. If you changed your name to the previous marriage then that is your legal name unless your divorce decree reverses it or you have gotten a court order name change.
Please fill out the "Tradename Form" to file a DBA/business. The cost to file is $10.00 cash or check. The form needs to be notarized prior to filing.
To cancel a DBA/business please fill out the "Cancellation Form". The cost to file is $10.00 cash or check. The form needs to be completed by the same parties that filed the original certificate. The form needs to be notarized prior to filing.
